In April, William, Patricia and Judith first opened Pathways to Health in Nesconset, NY and the vision became a reality. In that first year they jointly created over 12 empowering classes and certification courses. Spirit Journey became the foundation class and AN-RA energy based healing was introduced. Pathways Of Love & Light ( a healing multi-faith ministry est. 1993 by William Bezmen ) also grew and they began an ordination tract with their first ministers, Patricia Stringham, Ed Stringham and Judith Grant.

Pathways will serve a minimum of 3,250 individuals experiencing, or at-risk of experiencing, homelessness from November 2017 through December 2020. At full capacity, the program will have 1,000 individuals enrolled and receiving services on any given day.
The Celebration Of Light began that year honoring the Light in every Faith. It has continue to be celebrated every year on the second Friday of December. On the second Friday of every month, open AN-RA healings were held and are still held today in order to be of service to the community at no cost, consistently for over 24 years. As long as a physician has prescribed home health, Medicare, Medi-Cal and most other insurance will cover home health visits.
Home Health Aide - HHA
Added to the staff are highly educated techs and counselors to care for all clients as Pathways continues to offer therapy, classes, certifications and overall self empowerment they had visioned. Other services like acupuncture also add to a variety of healing modalities. Our family of services constantly evolves based on our clients’ needs, new technologies, and changes to Medicare, Medi-Cal and other health insurance. We have specialized programs for chronic illnesses including heart failure, COPD, and pneumonia. Assists with the orientation of other providers, such as new nurses, therapists, social worker, spiritual care counselors and any office staff, such as schedulers and new managers. Participation in these orientations is usually accompanied by patient visits.

Insurance Coverage and Qualifying for Home Health
For more than a decade, UniHealth Foundation has invested in programs that support people experiencing homelessness as they progress from hospital discharge to permanent housing. A wide variety of services and supports are required along the way, and UniHealth has identified many interventions that help make patients’ journey healthy and successful. In addition, Pathways To Health™ provides a variety of programs and services offered both in-house and on an outreach basis.

Pathways Healers retreat was held in the beginning of October and Patricia taught what was to be her last class with all of the healers surrounding her in love. On December 10th, 1999 Patricia Madigan Stringham transitioned into the Light with her husband, children and William and Judith at her side. She never was able to come to the new location but her teachings and love are with the center and it’s mission always. Thank you to all of the brave and dedicated healthcare professionals providing care in the Bay Area and around the world during this unprecedented global health emergency. Our mission is to support and facilitate activities that significantly improve the health and well-being of individuals and communities.
Pathways works to address the root causes of homelessness, including housing insecurity, behavioral health issues, addiction, unaddressed health conditions, and lack of community connection. The program targets and stabilizes individuals experiencing homelessness with complex needs and high utilization of health care and public safety services, freeing up scarce public resources needed to support the local economy. By connecting individuals to health and housing services, Pathways will help alleviate the negative impacts of homelessness for all Sacramento residents. Led by the City, the program brings together local hospitals, community clinics, health plans, homeless services and housing providers, first responders, and community-based organizations to create an integrated system of care. The program is the cornerstone of the City’s commitment to housing 2,000 homeless individuals by 2020.
Enrollments in the program on are on a first come, first serve basis and are based on verification of Pathways eligibility criteria. Pathways anticipates 50 percent of total annual enrollments from referrals from the Sacramento Police Department Impact Team, which targets homeless encampments and “hot spots” with a high level of need. The remaining Pathways enrollments will be allocated based on referrals from hospitals, other navigator programs, health plans, clinics, shelters and homeless service providers. Alignment of all homeless service programs, including emergency shelter beds, with Pathways wraparound services is a key goal of the City of Sacramento. Emergency shelter provides stability and opportunities for frequent contact with service providers that can be leveraged by Pathways, making it easier for the program to actively address individuals’ needs and connect them to housing. Other supportive services include comfort measures and integrative therapies, such as aromatherapy and Comfort Touch©™.
A new initiative between two Maryland medical and educational institutions will train members of underserved communities to become nurses. At Directions, we see our role as giving hope to people who sometimes feel they have none, and working with them, their family and friends to help them transform their lives for the better. We apply a holistic approach to our work and can tailor programs to suit the needs of each client. Directions Health Services have built a strong reputation among the sector and importantly with our clients for over forty years, as a provider who understands that everyone’s story is different. Serves as a resource to the new clinical provider, after orientation is complete.
Maintains and updates the level of knowledge and skills required to provide quality hospice aide care. Maintains communication with a case manager, patient, family and facility staff regarding all aspects of patient care. We work collaboratively with other nonprofit agencies, religious congregations, businesses, civic organizations, and individuals committed to solving child homelessness in Orange, Osceola and Seminole counties. Together, we help vulnerable children grow up supported and protected by their community. Pathways to Home brings long-term stability for the entire family, with childrens’ needs at the forefront. This year, 120 homeless families with children will build new lives with the support of the Pathways to Home program.
HOME INFUSION THERAPY
Mentors the clinical orientation of a new clinical provider per department orientation process, as assigned by the Home Health Aide Program Director or Home Health Aide Manager. Attends educational in-service trainings, department meetings and other agency meetings as requested by the manager. Attends and actively participates in the hospice interdisciplinary group meetings.
Pathways is one component of a comprehensive strategy initiated by the City of Sacramento to address homelessness. As a Medicaid program, Pathways will provide intensive, ongoing wraparound services to help enrollees access and maintain the housing options that are available, including shared housing, board and care, and publicly subsidized units. These supports help individuals stabilize and access housing options that are available as more housing capacity is developed.
